Action Items – Due April 7
Send a list of student & chaperone names attending the Field Trip to techoutreach@hyland.com. The list should also include the grade of each student attending.
Have Parents/Guardians complete the Parent/Guardian Consent Form for their student
Students cannot attend the field trip if they do not submit a completed form
Either a PDF version can be handed in on the day of the field trip, or an electronic form can be submitted by April 7
PDF copy is attached
Forms can be turned in upon your arrival (must submit all 3 pages for each student; printed front & back is allowed).
If you plan on bringing in completed printed copies instead, please let us know ASAP
